I further certify that I have been provided with educational material on the dangers and problems of drug and alcohol misuse.

I am fully aware, and agree that I may be discharged or otherwise disciplined for any violation by me of said DOT Drug and Alcohol Misuse Prevention Policy, for any failure or refusal to provide urine and/or breath specimens when requested by my employer, for the failure to identify and certify same, for the failure to cooperate with the forms and other documents, and/or for any other failure or refusal to cooperate with my employer in its said DOT Drug and Alcohol Testing Program.

AFFIDAVIT TO AUTHORIZE RELEASE OF CDL HOLDER'S REPORTED POSITIVE CONTROLLED SUBSTANCE TEST RESULTS

Use this form to obtain the CDL holder's reported positive controlled substance test results information.

This form should ONLY be used if you wish to inquire whether or not a prospective driver (CDL Holder) has had a positive controlled substance test result reported to the Texas Department of Public Safety in compliance with TRC 643.064 since 09/01/03.

THIS FORM IS NOT REQUIRED FOR REPORTING A POSITIVE CONTROLLED SUBSTANCE TEST.

1. This form must be completed in full and include the driver's original signature.
